1. Delegated Task: The ICB (Party 1) authorises the clinical task of [task name] to be performed by the Personal Assistant (Party 3) on behalf of the PHB holder (Party 2), subject to the conditions set out in this agreement.
2. Training Requirement: The PA must complete the relevant Meridian Care Academy course and receive nurse-assessed competency sign-off before performing the delegated task independently. Competency must be renewed annually.
3. Clinical Oversight: Meridian Care Academy provides a named nurse assessor responsible for competency sign-off and clinical oversight. The ICB retains clinical accountability for the delegation decision. The PHB holder accepts responsibility for ensuring the PA maintains training currency.
4. Escalation: The PA must escalate to [named clinician / 999] in the event of clinical deterioration, equipment failure, or any situation outside the scope of their training. The escalation pathway is detailed in the individual's care plan.
5. Review: This agreement is valid for 12 months from the date of nurse sign-off. It must be reviewed annually or immediately following any clinical incident, change in condition, or change of PA.
